Personal Nutrition Assessment and Consultation (1 hour):

The Registered Dietitian will ask a series of questions regarding dietary habits, lifestyle and activity while completing the nutrition assessment
Next, we’ll complete a 24-h Food Recall for more detailed diet assessment
The RD will put the assessment together to identify specific areas for improvement, followed by goal setting and recommendations to take home
Next, the RD will complete diet teaching for identified dietary issues accompanied by educational handouts (eg. weight loss, cholesterol lowering, healthy heart, diabetes, impaired kidney, low sodium, cancer, lactose intolerance, celiac disease, Crohn’s diet, IBS, dysphasia, allergies, etc)
Body composition analysis available upon request. Find out your body fat percentage, visceral fat, muscle mass percentage, and biological age with our new bioelectrical impedance scale. Extremely useful for tracking your weight loss and fitness goals.

Follow-up Sessions (30 min):

Recommended for everyone, but especially* for: diabetes, high cholesterol, high blood pressure, CHF, cancer/cachexia, weight loss and impaired kidney function since these problems often have measurable markers (eg. serum lipid profile, HgbA1C, Phosphate, Sodium, body weight)

Check on progress if measurable markers available (follow up with labs, weight)
Diet troubleshooting and solutions
Perfect/work on meal plan (if custom meal plan added)
Continue to work on specific goal setting
Reinforce dietary guidelines to ensure compliance
